Under the Affordable Care Act, states that expanded Medicaid cover adults with household income up to 138% of the Federal Poverty Level (FPL). States that did not expand Medicaid may have much lower limits or no coverage for childless adults. Income is based on Modified Adjusted Gross Income (MAGI).

Medicaid provides free or very low-cost coverage for low-income individuals and families. CHIP covers children in families that earn too much for Medicaid but can't afford private insurance. The Health Insurance Marketplace offers subsidized plans for those above Medicaid limits but below 400% FPL.

Does my state have Medicaid expansion?
As of 2026, 40 states plus DC have expanded Medicaid. The 10 non-expansion states are primarily in the South. In non-expansion states, adults without children or disabilities may fall into a "coverage gap" — earning too much for traditional Medicaid but too little for Marketplace subsidies.
What does Medicaid cover?
Mandatory benefits include inpatient/outpatient hospital care, physician visits, lab/X-ray, nursing facility services, and home health. Many states add dental, vision, prescription drugs, and mental health coverage. There are usually no premiums and minimal copays.
